The patient web portal offers the opportunity to complete surveys outside of the app. This can be particularly useful, if you do not have access to a smartphone or tablet, and so can not download the app. All app users can also utilise these online surveys via the patient web portal, however we would encourage patients to still use the app to access all of the educational materials and other features within the app. Please see further details below on how to access web portals.

 

How to access web portals if you have already created an account 

 

If you have already created an account, and would like to access the online surveys via the web portal, you can follow the link within your registration email. 

 

Now login for the online surveys using your username (which will either be your email address or phone number) and your password. 

 

You should now be able to access the available online surveys via the web portal. 

 

How to create an account online and access the web portals

You may have a link in your invitation email which allows you to create an account outside the app on a web browser.

 

After creating your account we would recommend you download the app and login by tapping “I’ve already created my account online”


If you do not have access to a smartphone or tablet, and therefore cannot download the app, you can still access some of the online surveys by clicking “Access web portal” in your web browser.



 

You should now be able to access the available online surveys via the web portal. 

 

How to access web portals if you do not have an email address or phone number

 

If you have not registered for the app, then you can still complete the online surveys by registering using both your:

 

  • NHS number
  • date of birth

 

You can still access some of the same materials by clicking “Access web portal” in your web browser.

 

 

Once you are logged into web surveys, you should be able to view any surveys that require completing. These surveys will form part of a wider effort to review and maintain the quality of care for patients like yourself.
